About Us

New Horizons, Inc. is a 501(c)(3) non-profit organization that has three divisions – New Horizons Village, Cherry Brook Health Care Center, and New Horizons Middlebury. New Horizons, Inc. provides affordable housing, accessible transportation, other support services, short-term and outpatient medical rehabilitation, and long-term skilled nursing in supportive communities of care that enable seniors, adults with disabilities, and individuals facing challenges to achieve their wellness goals and live life as independently and joyfully as possible. We are shaped by the population we serve. Over half of our Board of Directors is comprised of people who have physical disabilities.
